It has come to our attention that some users who redeemed Tier 7 show a delivery status of "Pending" in their Purchase History and that these rewards have not been delivered. We are currently looking into the issue and will be re-issuing items for delivery as necessary after the end of the promotion.

Instructions: Please fill out once for every time you redeemed. E.g. If you redeemed Tier 7 once and up to Tier 5 a second time, fill the form out to tier 7 once, then a second time up to tier 5 (ignoring tier 6 and 7 the second time.)
